ResponsibilitiesCoordinates and oversees all aspects ofpatients’ home care.

Collaborates with patient care teams to developand implement care plans.

Educates families on patient condition,needs and care.

Performs head to toe physical assessments.

Checksvital signs and manages medications.

Gives injections, IV therapy ortube feedings.

Administers and maintains catheters.

Completesdocumentation timely and accurately.

Supervises LPNs andHHAs.

Regularly communicates patient progress to the clinicalmanager, physician and care team.

Other duties asassigned.

QualificationsOne year of clinical experience as aregistered nurse (RN). If less than 1 year clinical experience as aRN, candidate must be approved by VP clinical.•Current RN license,specific to the state(s) you are assigned to work.

Minimum sixmonths home health experience.

Current CPR certification.

Validdriver’s license, reliable transportation and liabilityinsurance.

Able to assess patient status and identify requirementsrelative to age specific needs.

Must be familiar with general useand functions of the computer, such as, user names and passwordconcepts; internet; e-mail; navigation of computer desktop,including starting programs, using files and windows, effectivelyuse navigation buttons and tool bars; ability to self-manage onlineHR services and online training programs.•Note – If less than 6months clinical experience as a RN, candidate must participate inRN Intern program.
