Required Qualifications:

Oneyear of recent experience as a Registered Nurse, including aminimum of six months in a Home Health setting as an RN Proficientin OASIS documentation Valid RN license in compliance with stateregulations BLS certification (AHA/ARC)

Preferred Qualifications:

Home Health Nursing Certification (RN-BC) Other certifications andlicenses may be required for this position

Summary:

A Home HealthRegistered Nurse (RN) delivers direct nursing care to patients intheir homes, ensuring comfort, stability, and effective managementof their medical conditions. Home Health RNs assess patientconditions, develop care plans, administer treatments, and educatepatients and caregivers on disease management.

Essential WorkFunctions:

Assess patient conditions and develop individualizedcare plans based on medical needs and physician directivesAdminister medications, treatments, and wound care while monitoringpatient responses Provide patient and caregiver education onmedication management, disease progression, and self-caretechniques Monitor for changes in patient status, reportingconcerns and adjusting care plans as needed Coordinate withphysicians, therapists, and other healthcare professionals toensure seamless care transitions Document patient care,interventions, and progress accurately and in compliance withregulatory standards Ensure compliance with infection control,safety, and home care best practices to protect patients andcaregivers Support families and caregivers, offering guidance andemotional support throughout the care process Maintaincommunication with case managers and interdisciplinary teams toprovide coordinated patient care Strong ability to completecharting and documentation requirements while meeting visit andproductivity goals Adhere to infection control, safety, andhealthcare regulations Perform other duties as assigned within theRN scope of practice and home health regulations

Required EssentialSkills:

Critical thinking, service excellence and goodinterpersonal communication skills The ability to read, write, &communicate in the English language is required Ability toread/comprehend written instructions, follow verbal instructions,and proficiency in PC skills Physical Abilities

• Remain in astationary position, move about, move equipment (50-100lbs),pushing, pulling, bending Interpersonal Skills
• To workeffectively with a variety of personnel (professional andancillary) to present a positive attitude and professionalismTechnical/Motor Skills
• Ability to grasp, fine manipulation,pushing/pulling and able to move about when assisting withprocedures and/or using department equipment. Mental Requirements
• Must be able to cope with frequent contact with the general public,customers, and meeting deadlines under pressure. Must be able towork under close supervision occasionally as well as workingwithout assistance from other personnel.
