Job Description

RN Case Manager Radgov 851 Mcarthur Street, Manchester, TN 37355 Required to see patients daily and be able to help with on-call rotation. On-Call schedule is released 30 in advanced.

Shift and work schedule:

Monday thru Friday 8:00 am to 4:30 pm and participate in the on-call rotation Having many sites per day/visit (if applicable): approximately 25 visits per week Mileage reimbursement: $0.41 Wound Care Experience Required-No certification, just basic knowledge and skills HomeCare HomeBase (HCHB) -Required CPR/BLS License Valid DL and Insurance Overview We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring effective communication among healthcare providers, and facilitating discharge planning. The ideal candidate will possess strong clinical skills and a deep understanding of patient needs across various settings, including inpatient and ICU environments.

Responsibilities Primary Responsibilities:

Perform comprehensive assessment of high risk members. Collaborate with primary care providers to ensure the implementation of an individualized, comprehensive care plan with specific interventions designed to engage the member. Address, identify, and continuously reassess cost-efficient, appropriate levels of care. Put non medical support in place to ensure compliance with treatment plans (such as housing and transportation). Coordinate care transitions as needed. Coordinate with physicians, social workers, discharge planners, and claims professionals in transitioning patient to appropriate level of care. Engage specialty resources as needed to achieve optimal resolution. Keep detailed records of clinical, functional, and fiscal outcomes during the management process. Identify opportunities for health promotion and illness prevention. Prevent adverse patient occurrences when possible and intervene quickly if prevention is not possible, thereby minimizing poor outcomes. Act as patient advocate by protecting privacy and confidentiality issues.

Qualifications :

Associates or Bachelor?s Degree in Nursing or related field. Current, unrestricted state Registered Nurse license. Ability to analyze complex medical information and make rational decisions. Case Management certification.. Skills Strong knowledge of physiology and the ability to apply this knowledge in a clinical setting. Experience in inpatient care, particularly in ICU or PICU environments. Proficient in vital signs monitoring and interpretation. Excellent patient assessment skills to identify needs effectively. Familiarity with geriatric care principles is a plus. Strong communication skills to facilitate effective interactions with patients, families, and healthcare teams. Ability to work collaboratively in a fast-paced environment while managing multiple priorities.
