Job Summary:
Seeking a compassionate and experienced Registered Nurse for a Home Hospice Case Manager role. The RN will travel to patients’ homes to conduct comprehensive assessments, coordinate individualized care plans, and ensure continuity of hospice care. Collaborates with physicians, social workers, and other healthcare professionals to optimize patient outcomes and ensure quality end-of-life care.
Responsibilities:
Perform initial and ongoing patient assessments Develop and implement patient-centered care plans Coordinate services and care transitions Identify non-medical support needs (e.g., housing, transportation) Monitor clinical and functional outcomes Educate and support patients and families Maintain accurate documentation and uphold privacy standards
Qualifications:
