This position offers the flexibility of working remotely from home, except for scheduled in-home assessments to build your client base and a monthly company meeting (once per month). As an RN Care Coordinator, you would provide the following services for each client: Act as a primary point of contact for the client / family for nursing related questions/ concerns. “On-duty” hours for this position will be during normal business hours 7a-7p, during which time the client / family can contact you as needed. However, we do ask that you are readily available to take calls/emails during “on-duty” hours. Outside of your “on-duty” hours, communication will be limited to emergency situations only. Conduct the initial consultation and 90-day reassessments, including nursing assessment and care plan. Outside of the initial assessment and 90-day reassessments, the remainder of the job can be on the mobile device from home. Initial assessments are typically done within 24-48 hours of request, so you will need to have some schedule flexibility to accommodate short notice requests. Communicate with and supervise all CNA caregivers, providing direction and instruction as needed. Attend our monthly company meeting You will be tasked to manage the care process, instruct caregivers, find solutions to issues and concerns that family members have related to your client(s) plan of care, and generally ensure that the care process runs smoothly. Compensation You will be paid $150 per case on a bi-weekly basis. This covers all aspects of managing the case. You will start with up to 3-5 cases and increase as desired. You will be paid $50 per in-person assessment for travel You will be paid $50 per caregiver orientation class that you participate in. Initially biweekly increasing to weekly.
