Apply today! -Job SummaryThe Registered Nurse in Home and CommunityBased Services provides and directs provisions of nursingandpersonal care to patients in their homes as prescribed by thephysician or as authorized by the state/program andin compliancewith applicable laws, regulations, and agency policies. TheRegistered Nurse coordinates the plan ofcare with other health careprofessionals involved in care and helps to achieve and maintaincontinuity of patientcare by planning and exchanging informationwith physician, agency personnel, patient, family, case managerandother community resources. -Specific Job Duties/Responsibilities – Provides high quality clinical services within the scope ofpractice and within infection control standards, inaccordance withthe plan of care/service plan, and in coordination with othermembers of the patient/client’scare team from admit throughdischarge. Completes clinical nursing assessments in accordance withfederal and/or state program requirements and asrequired bypayer Ensures the patient/client’s eligibility and medical necessityfor services as defined by payer source andagency policy. Develops and revises individualized plans of care and/orservice plans according to federal and/or stateprogram requirementswith other community providers. Ensures plan of care frequency and duration meets the needs ofthe patient and initiates plan of carerevisions as needed withphysician approval.

Experience Desired A minimum of one year experience as an RN preferredLicenseRequirements Must have current RN licensure in state of practice. Current drivers license, vehicle insurance and access to adependable vehicle or public transportation.
