Delivers skilled nursing care in accordance with MD/FNP/PA orders in the home environment. Utilizes the Electronic Medical Record to document all care provided. Reports changes in patient condition to MD/FNP/PA and prepares orders to reflect these changes. Participates in staff meetings, in-service programs and patient care conferences. Participates in the on-call rotation. All other duties as assigned. Education & Experience for Home Health RN (Registered Nurse),

Current RN license to practice in the state of Tennessee. Valid TN driver’s license, current liability insurance and reliable transportation. Current CPR certification required. Home Health RN (Registered Nurse),
