Provides direct patient care to patients using the nursingprocess in accordance with applicable scope and standards ofpractice. Performs plan of care intervention, including medicationadministration, specimen collection, clinical treatments, as wellas other medical care treatment. Documents patient care given.

Monitors, records, and communicates patient condition asappropriate to care team, physician, patient, and family. Assistspatients with performing activities of daily living, includingpersonal hygiene, elimination, nutrition, and ambulation.

Collaborates as needed across disciplines to coordinate patientcare, including patient transfer, discharge, referral andspiritual/psychosocial support needs. Evaluates learning needs ofpatient and/or family and provides patient/family educationappropriate to age, culture, condition, and circumstances. Works asan advocate for the physical and emotional well-being of thepatient. Perform other duties as assigned. May be required toperform duties of the CNA, MA or LPN or other nursing staff onoccasion. Attends and participates in staff meetings, in-service,projects and committees as assigned. Adheres to and supportspolicies and procedures of the Highpoint Health with AscensionSaint Thomas. Works scheduled shifts including overtime, whennecessary. Accepts all call schedule as directed. Maintains a neatand professional appearance in compliance to the existing dresscode. While performing this job, occupational exposure tobloodborne pathogens exposure is present for all employees. Reportsto the Director of Skilled Care.

Qualifications:

Minimum EducationAssociate’s degree is required. Required Skills Requires criticalthinking skills, decisive judgment and the ability to work withminimal supervision. Must be able to work in a stressfulenvironment and take appropriate action. Certifications AHA BasicLife Support (BLS) within 30 days of hire. License Must be alicensed Registered Nurse with the State of Tennessee or a licensedRegistered Nurse from a Compact State with the proper Tennesseepaperwork filed within 30 days of hire.
