ResponsibilitiesThe Registered Nurse (RN)in Home Health provides and directs provisions of nursing care topatients in their homes as prescribed by the physician and incompliance with applicable laws, regulations, and agencypolicies.

Coordinates total plan of care with other health careprofessionals involved in care and helps to achieve and maintaincontinuity of patient care by planning and exchanging informationwith physician, agency personnel, patient, family, and communityresources. All done within a Point of Care setting.

Makes theinitial and/or comprehensive nursing evaluation visit, determinesprimary focus of care, develops the plan of care within statespecific guidelines, and submits accurate, complete, and timelydocumentation in collaboration with physician orders.

Performspatient comprehensive assessments at designated time points anddevelops the appropriate plan of care.

Ensures patients meet homehealth eligibility and medical necessity guidelines as defined bypayer source.

Education And ExperienceExperience Requirements Prior to RN making a psychiatric nurse visit, the agency willadhere to Psychiatric Skilled Nursing/Scope of Care Must have experience within the last five years. If not,documentation must support psychiatricretraining classes or CEUs toupdate psychiatric knowledge.

License Requirements Must have current RN licensure. Current CPR certification required. Must have a current drivers license and vehicle insurance, andaccess to a dependable vehicle, or public transportation.
