This RN Case Manager Position Features:
I Am Guardian Home Care? As a RN Case Manager you will: Assess all medicines to identify possible ineffective drug therapyor adverse reactions, significant side effects, drug allergies, andcontraindicated medications. Establish realistic measureable, observable goals consistent withthe patient?s diagnosis. Comply with applicable legal requirements, standards, policies andprocedures including, but not limited to
Compliance Process:
Codeof Conduct, HIPAA, and documentation standards. Demonstrate professional conduct, service excellence and commitmentto the values and mission of Guardian Home Care. Requirements of Qualified Candidates Graduate from an approved school of professional nursing andcurrently licensed to practice as a registered nurse in the stateof agency operation One (1) year experience as a RN Must possess and maintain valid CPR certification while employed ina clinical role Has sufficient endurance to perform tasks over long periods of workhours Must be a licensed driver with an automobile that is insured inaccordance with state and/or organization requirements and is ingood working order.
