Job Summary:
The In-Home Wound Care Nurse Practitioner provides advanced wound management services to patients in their residences. The NP performs comprehensive wound evaluations, initiates treatment plans, conducts procedures such as debridement, and collaborates with interdisciplinary teams to promote optimal healing. This position is ideal for clinicians who are autonomous, patient-centered, and experienced in managing complex wounds in home settings.
Key Responsibilities:
Perform in-home evaluations of acute and chronic wounds (e.g., pressure ulcers, diabetic foot ulcers, venous/arterial wounds, surgical wounds). Conduct wound measurements, staging, and detailed documentation. Perform wound-related procedures including sharp debridement, application of advanced dressings, and use of negative pressure wound therapy (NPWT). Develop and update individualized wound treatment plans based on clinical findings and evidence-based protocols. Prescribe appropriate medications, dressings, and durable medical equipment (DME). Order and interpret relevant diagnostic tests (e.g., wound cultures, labs, imaging). Educate patients, families, and caregivers on wound care, prevention, and follow-up instructions. Coordinate care with primary care providers, specialists, home health agencies, and other clinical support staff. Accurately document all visits and procedures using an EMR system in accordance with compliance standards. Monitor healing progress and adapt interventions to optimize outcomes. Participate in wound care quality initiatives, staff training, and clinical case reviews.
Qualifications:
Master’s or Doctorate in Nursing (MSN or DNP). Current and unrestricted Nurse Practitioner license in Tennessee Board-certified NP (FNP, AGNP, or equivalent). Strong knowledge of wound care products, protocols, and documentation requirements (e.g., Medicare guidelines). Excellent clinical judgment, time management, and independent decision-making skills. Valid driver’s license, reliable transportation, and willingness to travel within service area. Certification in Wound Care (WCC, CWCN, CWS, or WOCN) strongly preferred. Minimum 2 years of experience as an NP, with at least 1 year in wound care or home health settings preferred.
