Monitors and manages patient care to promote continuity ofcare, optimal patient outcomes, patient satisfaction, costefficiency, and compliance. Consults with nursing staff andmultidisciplinary team regularly to evaluate patient’s status andappropriateness of medical care, including admission, length ofstay, transfer and discharge. Monitors patient and familysatisfaction. Responds to questions and complaints from patients,family members, and payers regarding care. Participates indischarge planning including coordinating patient transfers toother facilities and coordinating community resources. Providesdischarge education and resource referrals to patients. Performschart review to identify actual or potential issues with servicedelivery, patient outcomes and satisfaction, compliance, cost, andreimbursement. Perform other duties as assigned. May be required toperform duties of the CNA, MA or LPN or other nursing staff onoccasion. Attends and participates in staff meetings, in-service,projects and committees as assigned. Adheres to and supportspolicies and procedures of the STRHS. Works scheduled shiftsincluding overtime, when necessary. Accepts all call schedule asdirected. Compliance to the STRHS Standard of Behavior is required.

Qualifications:
Minimum Education Associate’sdegree is required. Required Skills Requires critical thinkingskills, decisive judgment and the ability to work with minimalsupervision. Must be able to work in a stressful environment andtake appropriate action. Certifications AHA Basic Life Support(BLS) Licenses Registered Nurse licensed by the State of Tennessee,or licensed by a Compact state and proper request for Tennesseelicensing have been submitted.
