$1,200 Day shift $1,750 Nightshift Maxim Healthcare is hiring for a Registered Nurse (RN) towork on our Transition team. Our Transition Team is a supplementalcare unit of skilled nurses whose primary function is to quicklyand efficiently get patients home while primary nursing staff areactively recruited. This team is deployed after we receive apatient referral for home care services and the patient is admittedto Maxim. Transition Team nurses provide care for all authorizedhours in a home during a provisional period between 1-90 days.

During this time, our office staff will be recruiting to identifyfull-time, replacement home healthcare nurses. The new caregiverswill have an opportunity to train and shadow in the home with theTransition Team caregivers prior to working independently with thepatient. Once the replacement nurses are trained and have aschedule, the Transition Team are then transitioned to another newadmission. Why Join Maxim? Responsibilities:

Utilizes the nursing process to plan, implement,and evaluate patient/client care and/or services provided toclient(s) receiving contracted services, as required Initiates theplan of care when directed and requests from prescriber anynecessary revisions as patient status and needs change Incorporatestherapeutic, preventive, and rehabilitative nursing actions basedon patient diagnosis and established goals Intervenes as applicablein response to changes in patient/client condition and reports suchchanges to appropriate physician or supervisor Administersmedications and treatments as prescribed, as well as, monitors anddocuments effectiveness of intervention Reviews plan of care andprogram protocols and identifies areas for healthcareteaching/education, care coordination, goal setting and dischargeplanning needs, as applicable

Requirements:

Current RegisteredNurse (RN) License for the state in which the nurse practices.

Current Health Certificate (per facility Registered Nurse (RN)contract or state regulation). Current PPD or Chest X-Ray. CurrentBLS card. One year prior Registered Nurse (RN) experiencepreferred.
