Responsibilities:

• Utilizesthe nursing process to plan, implement, and evaluate patient/clientcare and/or services provided to client(s) receiving contractedservices, as required
• Initiates the plan of care when directedand requests from prescriber any necessary revisions as patientstatus and needs change
• Incorporates therapeutic, preventive, andrehabilitative nursing actions based on patient diagnosis andestablished goals
• Intervenes as applicable in response to changesin patient/client condition and reports such changes to appropriatephysician or supervisor
• Administers medications and treatments asprescribed, as well as, monitors and documents effectiveness ofintervention
• Reviews plan of care and program protocols andidentifies areas for healthcare teaching/education, carecoordination, goal setting and discharge planning needs, asapplicable

Requirements:

• Current Registered Nurse (RN) Licensefor the state in which the nurse practices.
• Current HealthCertificate (per facility Registered Nurse (RN) contract or stateregulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior Registered Nurse (RN) experience preferred.
