Job:
PURPOSE STATEMENT

The Intake RN performsassessments at or outside the facility, depending on the needs ofthe patient or hospital. Screens and assesses admissions toevaluate clinical and medical appropriateness and communicates thisinformation to a physician who makes the admissions and level ofcare decision. Works closely with business office, nursing, andclinical staff as well as external parties to ensure the timelyadmission of patients. Responds to inquiries regardingadmissions.

Roles and Responsibilities:
ESSENTIALFUNCTIONS

Facilitates admissions and assessment processes forincoming patients. Provides initial evaluation and initiate theintegrated assessment, documenting patient symptoms, functioningand needs.

Collaborates with other facility medical and psychiatricstaff to ensure appropriate recommendations and admissions.

Communicates projected admissions to designated internalrepresentative in a timely manner.

Ensures all medical admissiondocumentation is gathered from external sources prior to patientadmission and secures initial pre-authorization for treatment andadmission.

Reviews patient charts to determine medical necessity andstaffs each referral and assessment with appropriate physician toobtain orders, recommendations, and DSM-V diagnosis, whenappropriate.

Communicates treatment recommendations to andcoordinate with other treatment teams.

May administer patientsmedications and follows all physician’s orders in consultation withpsychiatrist and in accordance with established hospital andlicensing agency protocols. Educates patients on side effects, andthrough discussions determines the effectiveness of education;questions compliance and adjusts medications under psychiatrists’supervision as needed; monitors lab results to validate medicationcompliance.

May maintain accountability of administration,recording, control and security of medications, includingnarcotics. Assures completion of Adverse Drug Reaction forms asnecessary. Maintains knowledge of the most common medications usedfor psychiatric patients.

Advocates for the lowest level of careconsistent with the patients needs. Coordinates admission andtransfer between levels of care within the facility.

Assuresthorough, accurate documentation is completed for all patients oneach shift and facilitates the response to requests for services,including the initial response, the assessment, and referralprocess, the designation of appropriate level of care,initiation/admission into services, and appropriate follow upactivity.

Responds to inquiries about the facility within facilitypolicy timeframes.

Performs insurance benefit verifications,disseminating the information gathered to patient, their familiesand other appropriate facility staff.

Provides accurate and ongoingassessment of patient status in the admissionsprocess.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS

Graduation from anaccredited school of nursing required. BSN preferred.

One or moreyears’ experience in mental health or substance use disorderadmissions preferred.

Experience in clinical interviewing, patientassessment, referral, treatment planning, communicating withexternal review organizations or comparable entities.

Experience inthe managed care pre-certification process, level of careassessments and utilization.

Outstanding interpersonal andinterviewing and assessment skills. Skill in telephone etiquetteand paging procedures.

LICENSES/DESIGNATIONS/CERTIFICATIONS

Validcurrent RN license from the state or from a compact state, whereapplicable.

CPR and de-escalation and restraint certificationrequired (training available upon hire and offered byfacility).First aid may be required based on state or facilityrequirements.
