In Home Nurse Family Hope Healthcare LC College Grove, TN 37046 Job Summary We are seeking a compassionate and skilled Home Care Nurse to provide high-quality nursing care to patients in their homes. The ideal candidate will have a strong background in various medical fields, including pediatrics, internal medicine, and outpatient care. As a Home Care Nurse, you will play a crucial role in enhancing the quality of life for patients by delivering personalized care tailored to their individual needs. Duties Conduct comprehensive assessments of patients’ health and medical needs in their home environment. Develop and implement individualized care plans based on patient assessments and physician recommendations. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Monitor patients’ vital signs and health status, documenting changes and communicating with healthcare providers as necessary. Provide education and support to patients and their families regarding health management, medication administration, and lifestyle modifications. Collaborate with interdisciplinary teams to coordinate care for patients in assisted living or home community settings. Perform basic life support as needed during emergencies or health crises. Maintain accurate records of patient care activities, including coding for services rendered and laboratory results. Experience A valid nursing license with experience in home community care Experience working in hospitals or assisted living facilities is a plus. Proficiency in using healthcare systems for documentation and coding purposes is desirable. Strong communication skills to effectively interact with patients, families, and healthcare professionals.
