POSITION SUMMARY Remote RN•Home Health & Poly-Chronic Disease Management
POSITION DUTIES
Assist Poly Chronic patients with Medication review, Care Plan Development, Disease process education Coordination of care Making appts., etc.
MINIMUM REQUIRED QUALIFICATIONS
2+ years RN experience. Active and unencumbered RN licensure in state of assignment or compact licensure where applicable in state of assignment.
PREFERRED QUALIFICATIONS
Experience with Poly Chronic disease management, CKD, CHF and dynamics associated. Ability to make recommendations and educate patients on health related choices associated with plan of care. Experience with In Home Care management, and in home Dialysis, are strongly preferred
