Registered Nurse :
This RN Case Manager will conduct home visits to provide patient care. The RN Case manager will work alongside an NP, RN team, Social worker, and other case managers to collaborate and promote ongoing care Provide wound care, monitored vitals, administered medications, and performed death pronouncements as needed.
Schedule:
Monday
- Friday, 8:00am
- 5:00pm On-call one night per week from 5pm-11pm (no weekends) Required Qualifications for Hospice RN Case Manager /
Registered Nurse :
Active RN License in TN At least 2 years of RN experience Hospice/Palliative care experience Related experience in Hospice or Home Health or Oncology or ICU or Med/Surg or LTC/SNF Benefits for Hospice RN Case Manager /

Mileage reimbursement Full health benefits PTO
- 6.77 hours accrued per pay period (increased after 3+ years of employment)
- starts accruing day 1 Extended Sick Bank
- 2.77 hours per pay period

Pay Details:
$74,173
