An active member of the interdisciplinary healthcare team the CaseManager facilitates internal and external delivery of healthcareservices. The Case Manager integrates the roles of UtilizationManagement, Case Management and Discharge Planning. The CaseManager is responsible for care coordination and development of thedischarge plan through collaboration with the healthcare team andinternal/external resources. The job objective is to promote acollaborative interdisciplinary care delivery model to achieveoptimal clinical and financial outcomes with adherence toregulations and compliance.

Qualifications:

Associate’s degree x Required Bachelor’s degree x Preferred Required Skills

• Ability to understand, communicate, and react effectively topatient and family needs
• Critical thinking
• Work well independently while keeping calm and maintainingcustomer focus
• Must be able to work collaboratively with internal and externalresources for optimal discharge planning Required Licenses Registered Nurse
• Current/active North Carolina or compact RN license
• BLS optional
