INTAKE COORDINATOR
The Coordinator is responsible for receiving all new patient referrals and intake information as well maintenance and upkeep of scheduling records and logbooks, and the accurate and timely communication of scheduling changes between office and field staff. Qualifications
- Two (2) years experience as Coordinator or in another health-related field
- Ability to manage routine office files, log books, and field staff schedules
- Must possess positive, enthusiastic attitude, good people skills, and professional appearance
- Ability to work independently without constant supervision
- An LVN/LPN with home health background is also acceptable Responsibilities
- Work directly with the All-County Health Care, Inc, Marketing Specialists regarding Initial screening of patient and understanding the patient’s physician orders.
- Calls physician’s office and hospitals to obtain clinical documentation required.
- Identifying potential medical/ social problems for the visiting RN prior to the SOC assessment.
- Validate Patient’s Medicare Insurance eligibility and homebound status.
- Assisting with patient phone calls and questions.
- Maintaining accurate and timely documentation.
- Completes intake form, routes it to the appropriate Case Manager for admission approval.
- Communicates with Patients/families on Home Health Aide-related issues.
- Prepares weekly schedule for field staff and copies for distribution.
- Assists in coordinating services provided to patients
- Controls and monitors schedule changes.
- Checks compliance of visits done (on master schedule) after all notes are matched to charges; immediately brings any scheduling problems to the supervisor’s attention.
- Calls hospital Social Services department to notify them that an agency Patient has been admitted to their hospital.
- Gives and takes report from the on-call coordinator.
- Attends case conferences as requested by his/her supervisor.
- Obtains weekly visit count and reports results to the DON and Administration.
- Assists in relaying messages to field staff, office staff and community liaisons.
- Performs other duties as assigned by the Supervisor or Administrator.
- Maintains current Patient roster with necessary information.
- Projects a concerned, professional attitude toward departmental personnel.
- Works to develop a positive rapport with all staff members and community resources affiliated with home health care services.
- Maintains high visibility and availability while in the office. Organizational Relationship
