We have an exciting opportunity for you! We are hiring an Admission Coordinator and Preceptor to join our Admissions team. In this role, you will provide coordination of the Admissions RN’s/SW’s as related to the BBH Admission Process. Responsible for educating patients/families about Big Bend Hospice and the hospice benefit, evaluating and enrolling patients as appropriate into hospice services, identifying patient/family needs and barriers to admission. Manage the assignment of referrals to RN’s and SW’s as appropriate and addresses any barriers to the admissions process.

Travel Requirement :

This position requires travel throughout the Big Bend Hospice service areas.

Duties/Responsibilities:

(Includes, but is not limited to) Precepts clinical staff in the following: patient assessment, hospice/palliative care, and the care planning process with an emphasis on pain and symptom management and specific competency requirements. Manages the dissemination of information as requested to the Admissions Team. Additionally, provides information about Big Bend Hospice [BBH] services to all requesting it. Serves on the bench for hospice admission to ensure patients are taken on to service same day. Precepts admission nurses, ensures their successful transition from date of hire through 3-month Orientation period. Participates in the development, revision and successful implementation of ongoing education and coaching to existing clinical staff. Provides direct patient care periodically to maintain skill sets and competence, as well as maintaining familiarity with the demands of direct patient care. Performs field observation of clinical staff for the purpose of coaching computer use, coaching on how to conduct an efficient model hospice nurse visit, patient assessment, and care planning in hospice/palliative care, IDT model of care/care coordination, clinical/technical skills, and time management skills. Serves as the Clinical Resource for the Admissions team to help in expeditiously moving the referral to an admission; and guiding other Admissions Specialists through the process. Performs review of all documentation to ensure the patient’s chart is ready to be sent to billing. Provides team specific training of new employees (mentorship) to provide a consistent process and ensure positive performance. Other duties as assigned.

Education and Experience:

Associate degree in Nursing. A clear/active Florida Registered Professional Nurse. Three years of nursing experience including 2 years of “hands on” clinical experience such as providing care, supervising nursing services or teaching nursing skills in an organized curriculum or in-service program. Must have 1 year of nursing experience in hospice We Got the
