Job Description
Overview We are seeking a Rehabilitation Intake Coordinator, sometimes referred to as a Rehab Admissions Coordinator, Intake Coordinator – Rehabilitation Services or Rehab Intake Coordinator in our Physical Therapy department. The Rehab Intake Coordinator will manage and coordinate the patient referral and intake process for rehab services, ensuring timely, accurate, and efficient access to care. The Intake Coordinator will also ensure smooth onboarding of patients into physical therapy by managing referrals, verifying insurance, scheduling evaluations, and coordinating with staff and patients to support timely and compliant care delivery.

Referral Management:
Review referrals, verify medical necessity for physical therapy, and communicate with referring providers to clarify or obtain required information
Patient Intake & Scheduling:
Collect demographics, insurance, and prescriptions, schedule evaluations, and prioritize patients based on urgency and therapist availability
Insurance Verification & Authorization:
Verify insurance coverage, obtain pre-authorizations, and collaborate with billing to ensure clean claims and reduce denials
Communication & Coordination:
Act as liaison between patients, providers, and staff, educate patients on intake and expectations, and coordinate accommodations or interpreter services
Documentation & Compliance:
Ensure accurate, compliant intake documentation and maintain patient confidentiality according to HIPAA and hospital policy Qualifications Before we go any further, we do have some deal-breakers.
You must have:
Graduate from an accredited school of nursing, physical therapy, occupational therapy, or speech pathology Current Georgia license in nursing, physical therapy, occupational therapy, or speech pathology BLS upon hire and maintain current, Health Care Provider (AHA) Additional Qualifications That Are a Plus Two (2) years Rehabilitation/Case Management experience preferred Full benefits at
