Full Job Description General Purpose:

Manage the registration process of patients or clients referred to home healthcare for skilled nursing, therapy and home health aide services.

Essential Functions:

Provide centralized intake and client service functions in order to gather pertinent patient/client, clinical and financial information patients/clients requiring home health care. Enter referral information into the software system including clinical information and orders for services if available. Identify potential payer sources and verify insurance. May obtain ongoing authorizations for visits based upon written information supplied by clinical field staff. This position involves daily interaction with other office support partners and serving as the point of contact with clinical staff, referral sources and patients. May have access to personal health information (“PHI”) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.

Minimum Education & Experience Requirements:

Associates Degree or equivalent years of training or work experience Prefer a minimum of 2 years in a home health setting Knowledge of Medicare coverage regulations, home health, and Federal Laws governing Medicare Certified Home health. One (1) year of scheduling experience in a home healthcare organization setting HCHB experience a plus Ability to communicate both orally and in writing, with clients, clinical and non-clinical personnel. Excellent organizational skills Exceptional customer service and problem-solving skills Ability to multi-task, work under pressure with changing priorities and short deadlines and effectively handle a heavy workload. Proficiency in communication technologies (email, cell phone, etc.) Highly organized with ability to keep accurate notes and records.

Working Conditions & Physical Effort:

Work is normally performed in a typical interior/office work environment. Ability to work flexible schedule and/or evening hours as needed. Frequently walking or standing or sitting most of the time.
