Responsibilities The Central Intake Coordinator in Home Health provides administrative support to Central Intake Teams by adequately triaging incoming calls and entering referrals in operating system. Receives and communicates accurate referral information within Central Intake as well as to Branches. Education and Experience License Requirements Current LPN licensure in state of practice. Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation. CPR Certification required. Additional State Specific Requirements Current LPN Licensure in LA without restrictions. At a minimum, one year of experience working as an LPN.
