Job Summary The Intake Coordinator will be responsible for managing the intake process for clients seeking health services. This role involves gathering and assessing client information, coordinating appointments, and ensuring a seamless transition into our programs. The ideal candidate will possess excellent organizational skills, compassion, and a commitment to serving the community. Key Responsibilities Conduct initial assessments to gather relevant information from clients seeking services. Verify client eligibility and determine appropriate service pathways. Schedule appointments and coordinate with various departments to facilitate client transitions. Maintain accurate and up-to-date client records in accordance with confidentiality and compliance regulations. Provide clients with information on available services, programs, and resources. Work collaboratively with healthcare providers and community organizations to ensure a comprehensive approach to client care. Address client inquiries and concerns in a timely and empathetic manner.
