Homecare Agency Intake Coordinator Position Available In Middlesex, New Jersey

Homecare Agency Intake Coordinator Heritage Health and Home Care Cranbury, NJ 08512 Review all referrals that come in. Give a copy of any referral that we could accept to the DON for approval Develop and maintain a relationship with existing and new client’s case manager Obtain all client information and initiate a referral in the system with as much information as possible. Ensure that the client meets all requirements for admission Face to Face with the physician within the past 90-days Care Plan from the payer source noting the services and hours we will be providing Signed order from the physician Authorizations notified and okay given If HHA services, must have an available HHA to provide services on the Start of Care day. Keep a soft file of referrals we accept and when allowed scan all documents into their electronic chart. Notify DON and Scheduling when a referral is complete and ready to schedule the Start of Care. Track all referrals on a spreadsheet documenting if we accepted or not and if not why. Utilize an Excel spreadsheet to track all accepted referrals that includes all pertinent information. Track all authorizations and ensure that we receive the new authorization before the old one expires. Cover for HR and Scheduling when necessary. Will be on call as scheduled after hours, weekends, and holidays. Report any clinical issues to an RN. Report any patient complaints to the DON. Give scheduling report on all non clinical issues on the next business day including any missed visits. Contact other employees to fill any call offs.
