Position Summary The Intake Coordinator plays a critical role in the referral and admission process. This individual is the first point of contact for new referrals and is responsible for ensuring smooth communication between referral sources, families, and the clinical team. The role requires exceptional organizational, clerical, and interpersonal skills to support operational excellence in a fast-paced environment. Key Responsibilities Serve as a professional and compassionate first point of contact for patients, families, and referral sources Accurately process and document all new referrals Notify Community Representatives promptly upon receipt of a referral Alert the Clinical Director or designee regarding upcoming admissions Provide timely updates to referral sources throughout the intake process Track and follow up on key documentation, including: Skilled care status and discharge dates Receipt and completion of consents Initial Plans of Care, Physician Certifications, Revocations, and other admission documents Maintain a high level of accuracy and efficiency in administrative tasks Support cross-functional teams with timely communication and coordination Qualifications High school diploma or equivalent required Minimum of 3 years of relevant administrative or intake experience, preferably in a healthcare or hospice setting Strong communication and organizational skills Proficient in Microsoft Word, Excel, and PowerPoint Professional phone etiquette and strong typing skills Ability to manage multiple tasks simultaneously and handle sensitive information with discretion Why Join Grace?
