Full-time, Monday to Friday, 9:00 AM – 5:00

Position Summary We are seeking a highly organized, communicative, and detail-oriented Patient Care Advocate (PCA) to support our intake process and help seniors seamlessly access the care they need. The PCA plays a vital role as the liaison between our therapy teams, facility partners, physicians, and patients. This position is ideal for someone with experience in healthcare administration, benefits coordination, or patient intake who thrives in a fast-paced, mission-driven environment. Key Responsibilities Eligibility and Benefits Verification Confirm insurance coverage and verify patient eligibility for rehab services Document all benefits information accurately in the EMR system Authorization Management Obtain and track pre-authorizations or approvals from payers as needed Follow up proactively to ensure timely clearance for services Documentation Collection and Intake Support Gather all required intake documentation, including: Signed consent forms Physician’s orders Insurance cards and IDs Relevant medical history or prior therapy notes Ensure all documentation is complete and compliant before therapy initiation Communication and Coordination Serve as the primary point of contact for intake-related communications Coordinate with: Facility department heads and administrators Field therapists to collect outstanding documents or signatures Physician offices for referrals and clarifications Maintain positive, professional relationships with all stakeholders EMR and Records Management Input, track, and update patient information accurately in our records system Flag missing or incomplete documents and follow through to resolution Ensure timely documentation for audits, billing, and compliance Compliance and Policy Adherence Follow HIPAA guidelines and internal privacy policies at all times Stay up to date on payer requirements and documentation standards Additional Responsibilities Support scheduling coordination if needed during high-volume periods Participate in team meetings and contribute to process improvement initiatives Assist with outcome reporting and administrative support as needed Qualifications 2+ years of experience in healthcare administration, medical intake, or patient coordination (rehab therapy experience preferred) Working knowledge of insurance verification, authorizations, and healthcare compliance Excellent organizational and communication skills Ability to manage multiple tasks while maintaining attention to detail Proficient in using electronic medical records (EMR) systems Empathy, patience, and a passion for serving older adults Why Join Limber Therapies?
