With a goal of achieving excellence in everypatient and customer experience, the Central Intake Coordinator’score purpose is to review referral documentation to clinicallyassess and align the appropriate level of care, services andprograms with the goals of care for the patient based on theinformation received from the referral source, field TransitionalCare Coordinator and/or patient. The CIC will ensure completenessof the referral record, follow-up and collect missing referraldocuments required for HH regulatory compliance, communicate andcoordinate care with Case Management and field TCCs, obtain verbalorders when missing from initial referral documents, monitor faxqueue for documents received after initial referral is processed,maintain ownership of all agency referrals after initialprocessing, process and follow-up on transfer patients and managepatient PING database for HH admissions. The CIC will :
- Becomeeducated on levels of care and service across the healthcaresystem, care navigating as required in facilitating timelycoordination of certified or hospice care and services for patientsmoving from one level of care to another to ensure safe andeffective patient transition across the pot-acute continuum. Servesas a bridge between the SSO, healthcare team and the patient and/orcaregivers.
- Communicate and coordinate referrals and patient carewith the onsite TCCs and/or clinical areas to provide seamless careto patients. Acts as agency’s point of contact and liaison forother agency departments.
- Locate patients that transfer tohospital and communicate with clinical teams and TCCs.
- Receiveand resolve inquiries for referral data/items required for codingand billing.

- Efficiency and accuracy in completing workas assigned.
- Adherence to regulatory and agency policies andprocedures.
- This position is within our Homecare Customer ServiceDepartment. It is a clinical position with no face to face contactwith patients and referral sources.
- Accountable for teamperformance in achieving desired clinical and operationalperformance measures.
- Identifies and facilitates professionaldevelopment needs and competency for staff.
- Collaborate andcommunicate with Primary Care Providers and home care staff toensure continuity of medical care, to include obtaining,clarifying, validating service requests and completing verbalorders.
- Communicate with transitional care staff, clinicalcolleagues, physician’s offices, and home care staff to coordinatehomecare orders, follow up appointments, risk factors, insuranceparameters and goals of care.
- Ensure collection andappropriateness of referral documents to support sound medicalpractice.
- Reviews demographic and clinical pre-admissiondocumentation, ensuring accuracy of information. Reviews referringand transfer documents and medication list for accuracy andadherence to regulatory compliance and assuring the transitionalcare processes are implemented.
- Consistently communicates withHHCAH management to make sure all issues and problems areseamlessly handles so that both the patient and the referralsources are satisfied with the results and process.
- Providingconsultation to referral source on community resources and homecare issues.
- Ensuring adherence with referral managementprotocols, policies and procedures.
- Building and supportingpositive, effective relationships across the continuum and with thepatients and communities we serve.
- Utilizing sound clinicaljudgment identifying risk and safety concerns and triagingappropriately.
- Responding to internal and external communicationtimely and accurately.
- Acts as a liaison to
SSO, HHCAH
staff,departments and customers both internally and externally
- Locateand follow-up on transfer patients and communicates status toclinical teams and onsite TCCs
- Assist Homecare CustomerCoordinators with F2F requirements and MD verification, when workvolumes are high
- Adheres to the practice of confidentiality(HIPAA and other state/federal regulations) regarding patients,families, staff and the Agency
- Demonstrates H3W LeadershipBehaviors and supports the team in culture and team buildinginitiatives.
Qualifications Education:
LPN with an active licenseto practice in the state of CT. Bachelors degree preferred MinimumExperience 1. Minimum of three years nursing experience in clinicalspecialty area. Preferred Experience 1.
