This field-based role involves traveling to clients’ homes to conduct comprehensive intake assessments, educate new referrals about their rights and available services, and serve as the vital first point of contact in their home care journey. This position requires extensive daily travel throughout [service area] and operates with significant autonomy. Key Responsibilities Conduct in-home intake assessments for new client referrals within 24-48 hours of assignment Educate clients and families about their rights, available services, and care options in a clear, compassionate manner Complete comprehensive documentation including intake forms, care assessments, and service agreements Maintain strict confidentiality and adhere to HIPAA regulations at all times Coordinate with care teams to ensure seamless transition from intake to service delivery Manage daily schedule efficiently to maximize client visits while maintaining quality standards Provide exceptional customer service that reflects our company’s commitment to client dignity and care Submit accurate reports and documentation within required timeframes Participate in ongoing training to stay current with regulations and best practices
Required QualificationsNon-Negotiables:
Reliable personal vehicle (model year 2015 or newer) with current registration and inspection Valid driver’s license with clean driving record (no DUI/DWI in past 5 years, no more than 2 moving violations in past 3 years) Current auto insurance meeting state minimum requirements Smartphone with GPS capability and ability to use scheduling/documentation apps High school diploma or equivalent (Bachelor’s degree in healthcare, social work, or related field preferred)
Experience & Skills:
Minimum 2 years experience in healthcare, social services, or client-facing role Demonstrated ability to work independently with minimal supervision Excellent written and verbal communication skills Cultural sensitivity and ability to work with diverse populations Basic computer proficiency including email, databases, and mobile applications Strong organizational skills with attention to detail Professional appearance and demeanor

Compensation Details Base salary:
$37000 annually Performance bonuses: $4000 quarterly Mileage reimbursement: Current IRS standard rate ($0.67/mile as of 2024) Average annual mileage reimbursement: $3,000-$5,000 depending on territory Important Notes Background verification: All offers are contingent upon successful completion of background checks, drug screening, and reference verification Probationary period: First 90 days include additional training and performance monitoring Professional development: Ongoing education requirements include annual certification renewals and quarterly training sessions Equipment responsibility: Company-provided equipment must be maintained in good condition and returned upon separation We are an equal opportunity employer committed to creating an inclusive environment for all employees.
