infusion Patient care Medical terminology Communication skills Entry level Full Job Description Vital Care of North Augusta is seeking a professional, detail-oriented, and highly motivated Intake Coordinator to join our fast-paced specialty infusion pharmacy team. This role requires exceptional organizational, communication, and customer service skills to process the intake of referrals, coordinate with various departments, and support our patients with their specialty infusion needs. Responsibilities Accurately and efficiently enter acute and chronic referrals for specialty infusion medications. Communicate with patients/caregivers to gather necessary demographic, insurance and clinical information Manage faxes, upload documents, and maintain accurate patient records. Serve as a forward-facing contact for patients, providing professional support via phone and email. Answer incoming phone calls, address patient inquiries, and provide timely resolutions. Communicate and relay information to and from Account Executives, Pharmacy, Authorization Teams, Nursing, and referral sources. Ensure seamless coordination between departments to facilitate excellent patient care. Utilize knowledge of insurance basics and specialty infusion medications to assist with daily operations. Maintains confidentiality of patient and proprietary information
