Key Responsibilities:
Conduct assessments to evaluate patients’ needs, challenges, and goals. Develop personalized care plans or service plans tailored to patients’ specific needs. Provide referrals to appropriate community resources, including healthcare, mental health services, housing programs, and employment assistance. Offer emotional support and guidance to patients navigating difficult situations.
Qualifications:
Bachelor’s degree in Social Work (BSW), Psychology, Sociology, or a related field. Previous experience in case management, social services or direct patient support is preferred.
