JOB PURPOSE
The Pulmonary Navigator coordinates and manages outcomes of aspecific patient population to facilitate achievement of quality,services, and cost. Provides one to one education, telephonicdisease management services, and home visits to individuals withpulmonary diseases. Assigned to patient caseload from initialinpatient admission ensuring a smooth transitioning of care topost-acute care. Works collaboratively and maintains activecommunication with physicians, Care Coordinators andinterdisciplinary staff. Prepares, analyzes, and communicatesprofiles, reports, and summaries. Identifies and evaluates patientand family educational needs.

GD Qualifications:
MINIMUM EDUCATION REQUIRED
Graduate of an accredited school of Respiratory Therapy; Bachelorsdegree strongly preferred.
MINIMUM EXPERIENCE REQUIRED
Three years of recent and relevant clinical experience related torespiratory care. Prefer experience with multidisciplinary teams.
Experience with data analysis, Centers for Medicare/MedicaidServices, research, and process improvement.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW
Currentrespiratory care practitioner licensed by the State of Georgiaunder the Composite State Board of Medical Examiners, holdRegistered Respiratory Therapist (RRT), credential from NationalBoard for Respiratory Care (NBRC). Credentialed as PulmonaryDisease Educator via American Association for Respiratory Care(AARC) within 6 months of hire. Basic Life Support (BLS)certification.
ADDITIONAL PREFERRED QUALIFICATIONS
Advanced Cardiopulmonary Life Support (ACLS) preferred. Strongworking knowledge of healthcare systems and community resources.
Able to establish strong working relationships within a healthcaresystem.
