Salaried Speech Therapist (SLP), Home Health Full-time | Monday

Jefferson, Blount and St. Clair countiesPosition Overview The Speech Therapist is responsible for thedelivery of quality patient care according to the patient’sindividual physician orders. Care may involve assessments,administering prescribed treatments and therapies, care planning,communication with other members of the patients’multi-disciplinary team, supply management, emergency management,coordination of care, health promotion and teaching and trainingfamily members. Essential Job Functions:
  • Provide Speech Therapyservices as prescribed by the physician.
  • Follow policies andprocedures and conforms to all applicable patient care standards,licensure regulations and state laws.
  • Provide a writtenevaluation defining Speech Therapy needs, goals and expectedoutcomes in conjunction with referring physician’s Plan ofTreatment.
  • Provide a written Plan of Care for Speech Therapygoals and outcomes
  • Maintains the plan of care for assignedpatients according to identified Speech Therapy goals and outcomes
  • Participate in coordination of services (including dischargeplanning), with health care team, physicians, parent/guardians,nurses, and others.
  • Maintain patient and company confidentiality
  • Maintain effective communication between staff, health care teammembers, and family.
  • Serves as a resource consultant for clinicalstaff.
  • Provide physicians and Aveanna with written evaluation ofpatient’s needs, goals, and progress.

Requirements:
  • Master’s degree in Speech-LanguagePathology from an ASHA accredited college/university Current andunrestricted SLP license to practice in the state of practice
  • Current CPR certification
Preferred Qualifications:
  • One (1) yearof recent experience as a Speech Language Pathologist in an acutecare setting preffered HHH As an employer accepting Medicare andMedicaid funds, employees must comply with all health-relatedrequirements in all relevant jurisdictions, including requiredvaccinations and testing, subject to exemptions for medical orreligious reasons as appropriate.
