POSITION SUMMARY
Responsible to the Director of Professional Services, ClinicalManager to provide medically prescribed, goal-oriented therapy inaccordance with the Physician Plan of Care, Company Policies andprofessional Standards of Practice. Supervises personnel to delivercare and coordinates care delivery.
JOB RESPONSIBILITIES
Participates in the development of patient plans ofcare. Makes visits to the patients for assessments and evaluationsand communicates with the physician as needed. Implements the Planof Care. Documents the care and submits electronic documentation in atimely manner.
