Description Job Summary:
Responsible for the evaluation of patients, development, coordination, and implementation of the treatment plan; maintenance of informative, accurate and timely documentation; participation in team meetings; training of others as indicated; and participation in continuing education. Provides quality goal directed Speech and Language Pathology (SLP) evaluation, treatment, and discharge planning to patients with cognitive, communication and /or swallowing disabilities to improve optimal functional level of independence

Service Workers Essential Functions:
Performs initial evaluation within the specified timeframe
Performs objective evaluations properly according to instructions
Evaluates patient utilizing knowledge of the development needs appropriate to the age of the patient, pediatric, adolescent, adult, and geriatric patient
Evaluates patient utilizing knowledge and skills of the diagnosis for each patient
Re-evaluate the patient appropriately to adjust the treatment plan as needed
Establish measurable and appropriate long term goals, and short term goals utilizing knowledge of the diagnosis and developmental needs appropriate for the age and discharge setting of the patient
Treatment should be implemented within the specified timeframe and should be appropriate for the age, diagnosis, and goals set
Educates and counsels patients and families regarding treatment plans and progress
Treatment should be coordinated with team members and other medical needs
Performs quality patient care duties effectively and efficiently
Report significant findings by telephone to the physician
Records patient treatments on charts and provides information to the supervisor, Director, and physicians.
Plans and/or assists with patient discharge from therapy services
Supervises activities of clinical fellows, students, and other support personnel
Submits daily treatment documentation and charges in an accurate and timely manner
Serve as a interdisciplinary team member including but not limited to effective team communication adherence to team treatment plans, coordination of patient rehabilitation schedule, providing SLP expertise to team functions and providing staff education
Qualifications Education:
Master’s degree in Speech Pathology required
Completion of a one year clinical fellowship (CFY) required
Certifications:
ASHA certification preferred
Completion of American Heart Association (AHA) Basic Life Support (BLS) for healthcare providers course within 90 days of hire and to stay current
Licenses:
Current licensure to practice as a Speech Pathologist in the
State of Texas Experience:
One year experience as a Speech-Language Pathologist preferred in a rehab setting
Skills:
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision
Must be able to work in a stressful environment and take appropriate action
