Job Description Job Description BAYADA Home Health Care iscurrently seeking an experienced Speech Language Pathologist, SLP,for a per diem opportunity performing home health visits. Thisoffice services adult and geriatric patients in homes throughoutForsyth, Yadkin, Wilkes, Stokes and Surry counties. Prior home careexperience as a Speech Language Pathologist, SLP is a plus, but notrequired. As a home care Speech Language Pathologist, SLP, you willbe an integral member of a multi-disciplinary health care team thatprovides rehabilitative care and skilled nursing to clients,affording them the opportunity to receive the medical care requiredto remain at home.
Speech Language Pathologist, SLPResponsibilities:
- Make home visits to clients in designatedgeographic territories.
- Perform thorough evaluations to assess,diagnose, and provide therapeutic interventions for clients who areexperiencing speech, language, communication, or swallowingdifficulties.
- Develop a treatment plan based on the evaluation,physician’s orders, and recommended goals.
- Continually assess andrevise the speech therapy care plan, and participate with BAYADAclinicians in the multidisciplinary care plan, as appropriate.
- Educate and instruct patients, family members, or other patientrepresentatives, in rehabilitative care and activities necessary topromote the client’s health, safety, and independent living.
- Accurately document observations, interventions, and evaluationspertaining to patient care management and services provided,utilizing a state-of-the-art touch pad tablet.
Speech LanguagePathologist, SLP Qualifications:
- A current license as a SpeechLanguage Pathologist in North Carolina.
- Possess a certificate ofclinical competence in speech from the American Speech and HearingAssociation, or;
- Have the equivalent educational requirements, asindicated by school transcript or diploma, and work experiencenecessary for such a certificate, and be awaiting the opportunityto take the American Speech-Language-Hearing Associationexamination for certification, or;
- Have completed the necessarywork and education requirements, as indicated by school transcriptor diploma, and be in the process of accumulating the necessarysupervised work experience required for certification.
- Solidcomputer skills; prior experience with electronic medical records(EMR) preferred. Our employees are our greatest asset:
