Per Diem BAYADA Home Health Care is currently seeking an experienced SpeechLanguage Pathologist, SLP, for a per diem opportunity performinghome health visits for our office located in Concord, NH. Thisoffice services adult and geriatric patients in homes throughoutConcord and surrounding towns in New Hampshire. Prior home careexperience as a Speech Language Pathologist, SLP is a plus, but notrequired. As a home care Speech Language Pathologist, SLP, you willbe an integral member of a multi-disciplinary health care team thatprovides rehabilitative care and skilled nursing to clients,affording them the opportunity to receive the medical care requiredto remain at home.

Speech Language Pathologist, SLPResponsibilities:

Make home visits to clients in designated geographicterritories. Perform thorough evaluations to assess, diagnose, and providetherapeutic interventions for clients who are experiencing speech,language, communication, or swallowing difficulties. Develop a treatment plan based on the evaluation, physician’sorders, and recommended goals. Continually assess and revise the speech therapy care plan, andparticipate with BAYADA clinicians in the multidisciplinary careplan, as appropriate. Educate and instruct patients, family members, or other patientrepresentatives, in rehabilitative care and activities necessary topromote the client’s health, safety, and independent living. Accurately document observations, interventions, andevaluations pertaining to patient care management and servicesprovided, utilizing a state-of-the-art touch pad tablet.

SpeechLanguage Pathologist, SLP Qualifications:

A current license as a Speech Language Pathologist in NewHampshire Possess a certificate of clinical competence in speech from theAmerican Speech and Hearing Association, or; Have the equivalent educational requirements, as indicated byschool transcript or diploma, and work experience necessary forsuch a certificate, and be awaiting the opportunity to take theAmerican Speech-Language-Hearing Association examination forcertification, or; Have completed the necessary work and education requirements,as indicated by school transcript or diploma, and be in the processof accumulating the necessary supervised work experience requiredfor certification. Solid computer skills; prior experience with electronic medicalrecords (EMR) preferred.
