As a Home Health Speech LanguagePathologist , you will: Evaluate, direct and providespeech/language pathology service to patients in the home orfacility Participate in the development and periodic review of thePlan of Treatment and Plan of Care. Utilize professional skills andjudgment in assessing and treating disorders of speech, voice,language, hearing and swallowing to prevent, identify, evaluate andminimize the effects of such disorders and conditions. Administerand interpret diagnostic tests and applications of therapeutictreatments including audio logic screening. Observe, record andreport changes in the patient’s condition and response to treatmentto supervisor and/or the physician. Provide instruction andtraining to patients in use of alternative communication systemswhen appropriate. Provide counsel and instruction to patients,families and healthcare staff. Maintain and submit documentation asrequired by the Company and/or facility. Prepare and submit timelywritten reports of evaluations, visits, summaries, care plans, carecoordination activities and progress reports as required by Companypolicy. Participate in care coordination activities and dischargeplanning. Maintain the highest standards of professional conduct inrelation to information that is confidential in nature. Shareinformation only when the recipient’s right to access is clearlyestablished and the sharing of such information is clearly in thebest interests of the patient. Attend, participate in and/orconduct internal staff development programs, obtain continuingeducation as required by Company policy, regulation. Meet theeducation and experience requirements for Certification of ClinicalCompetence in Speech-Language Pathology or Audiology granted byASHA Minimum of six months experience as a speech therapist /speech-language pathologist Home Health experience a plus Currentand unrestricted license Current CPR certification Goodorganizational and communication skills Use your skills to make animpact

Required Experience/Skills:

Meet the education andexperience requirements for Certification of Clinical Competence inSpeech Language Pathology or Audiology granted by ASHA Minimum ofsix months experience as a speech therapist / speech languagepathologist Home Health experience a plus Current and unrestrictedlicense Current CPR certification Good organizational andcommunication skills A valid driver’s license, auto insurance, andreliable transportation are required. Pay Range • $49.00 – $69.00 -pay per visit/unit • $77,200 – $106,200 per year base pay ScheduledWeekly Hours 1 Pay Range The compensation range below reflects agood faith estimate of starting base pay for full time (40 hoursper week) employment at the time of posting.
