Assist the Community Access Manager in coordinating, supervising, and implementing community access services that promote meaningful and inclusive opportunities for individuals served. Support daily operations and team development in alignment with Cross Plains’ mission, vision, and values.

General Responsibilities:

Support supervision and coordination of DSP staff under the guidance of the Community Access Manager. Assist with onboarding, training, and coaching DSP staff. Facilitate communication between DSP staff, individuals served, and management. Participate in meetings to review individual support plans (ISP). Ensure timely documentation and submission of service hours to management. Assist with preparation and completion of required documentation, including Critical Incident Reports. Support adherence to DBHDD regulations and Medicaid compliance.

Community Access:

Help develop weekly schedules for DSPs to include volunteer activities, skill development, and inclusive recreation. Support DSPs in organizing and facilitating community-based events and activities. Research and identify community opportunities for engagement and inclusion. Assist in the coordination and facilitation of volunteer groups and community connections. Promote positive relationships with community partners and stakeholders.

Individualized Planning:

Conduct and assist with periodic surveys to gather feedback on services and individual interests. Support individuals in exploring and discovering new interests and community opportunities. Assist individuals in achieving their community inclusion goals. Advocate for individuals, ensuring they are treated with respect and dignity.

Leadership and General Duties:

Participate in training and development opportunities. Stay informed about organizational updates, policies, and procedures. Model and uphold CPCP values of respect, trust, and partnership. Attend internal and external meetings as required. Support the Manager in planning, coordinating, and attending events as necessary. Perform additional duties as assigned by the Community Access Manager.

Knowledge, Skills, and Abilities:

Associate degree or higher preferred, or equivalent experience in Human Services, Psychology, or related field. Experience in providing services in a human service environment preferred. Must pass criminal background check and drug screening. Strong communication skills (written and verbal). Proficiency in computer technology (Microsoft Office, data entry, documentation systems). Genuine care for individuals served, embracing their uniqueness. Valid Georgia Driver’s License.
