Full-Time Position Summary:
Oversees and provides quality member care, accountability, safety of members and staff; supervises weekend nursing staff (RNs, LPNs and CNA’s for the SNF (and AL on weekends). While on duty communicates with member’s families and Attending Physicians, after weekend rotation report to DON and AL Manager regarding concerns, incidents and member care of plan
Essential Duties and Responsibilities:
The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive.
- Direct the functions of the nursing staff (RNs, LPNs and CNA’s for the SNF (and AL on weekends ) to include but not limited to: ensure compliance with all current rules, regulations and guidelines that govern long term care facilities; ensure work assignments are performed within acceptable nursing standards; make reports/recommendations concerning the activities of staff.
- Conducts multiple rounds of both the SNF (and AL on weekends) addressing any issues or needs of members, families or staff
- Ensures appropriate staffing levels during the weekend, utilizes agency when necessary to meet the needs of the members and State regulatory requirements
- Creates detailed reports chronicling the weekend’s events (including staff, member, plan of care issues, incidents, etc.) to DON and Assisted Living Manager
- Works closely with Attending Physician reporting concerns, changes or issues with plan of care
- Maintains the Standard of Practice for Medication Administration as set forth by nursing governing bodies.
- Receives telephone orders from physicians and record on the MAR.
- Assures that appropriate, informative and descriptive documentation/medical records are maintained to include but not limited to: chart all accident/incidents involving members and notify physician; chart member’s admission, transfer and/or discharge; transcribe physician’s orders to resident charts, medications sheets and treatment/care plans; document unauthorized discharges in accordance with established department procedures; report all discrepancies noted concerning established member care to DON to ensure that the nurses’ notes reflect that the care plan is being followed.
- Coordinates nursing services portion of MDS and Member Care Plans/Discharge Plans with other involved department
- Ensures that narcotic records are accurate and notify DON of any discrepancies.
- Administers professional nursing services to include but not limited to: obtain sputum, urine and other lab tests as ordered; take and record TPR and blood pressure; tube feeding; suction; apply and change dressings/bandages, packs, colostomy and drainage bags; Range of motion exercises; care of dying.
- Participates in the orientation of new member/family members to the Health Care Center; continue to meet with members/family members as necessary.
- Monitors seriously ill members as necessary; notify the member’s attending physician and next of kin when there is an acute change in the member’s condition. Assists employees in solving questions and problems as needed as requested.
- Assists other employees in lifting and/or carrying unusually large or heavy objects each shift in a manner that complies with company safety procedures.
- Effectively communicates with members, families and Health Pavilion Staff
- Recognizes a change in a resident’s condition, acts appropriately to these changes, facilitating needed care.
- Communicates effectively in writing in patient records.
- Sets up and pass medications, observing the 5 rights of medication administration. Observes residents for drug effects/interactions. Order medications as needed, per pharmacy protocols.
- Greets all new members, completes admission paperwork, including admissions intake form.
- Facilitates timely discharges from the Skilled Nursing, providing written and verbal information to receiving facility or home care as needed.
- Attends and participate in continuing education programs designed to keep you abreast of changes as well as to maintain your license on a current status. Attend department, in-service and company meetings as scheduled.
- Maintains confidentiality of members’ PHI (Protected Health Information) and medical records information as well. This position is classified as a level 4 in the ePhi Access Group levels.
- This position is considered to be an essential role and the incumbent may be required to work during emergency situations including natural disasters.

- Completion of a nursing program at an accredited school of nursing.
- At least two years of experience in nursing care in a similar setting preferred; three years of supervisory experience required
- Possession of a current Florida nursing license
- Valid Driver’s License- The employee must have a valid driver’s license to drive their vehicle or a company vehicle to various worksite locations as required by the employer.
- Must have excellent people skills to relate effectively to staff, residents, families, and community members.
- Must have effective supervisory skills with the ability to participate in clinical and administrative decision making based on established policies and procedures.
- Must be empathetic, energetic and have an affinity for working with a diverse senior population.

Strong interpersonal, leadership, and motivational skills.
Technical Skills:
Knowledge of medical/nursing practices and procedures, laws, regulations, and guidelines pertaining to long-term care. Communicates effectively, both written and orally. Ability to perform multiple tasks simultaneously.
Physical/Sensory:
On a daily basis, throughout the workday: Must be capable of sitting, standing, walking, climbing stairs, reaching, turning, bending, stooping, crouching and kneeling, reaching over the head, grasping with both hands, and fine manipulation. Must have normal eyesight or use corrective lenses. Must have three-dimensional vision and ability to judge distances and spatial relationships so as to see objects where and as they actually are. Must be able to recognize colors. Must be able to hear and distinguish between normal tones and be able to perceive the nature of sounds. Must be able to exchange ideas by means of the spoken word as well as engage in activities to convey detailed or spoken instructions to other workers accurately, loudly, and/or quickly. Must have normal sense of touch and smell.
Environmental Adaptability:
Works in well-lighted, well-ventilated building. Must be able to tolerate occasional exposure to dust, fumes, odors, water, etc., as well as some noise. Subject to frequent interruptions. Subject to hostile or emotionally upset individuals. Must be willing to work, when necessary, beyond normal working hours and on weekends, as well as in other positions as necessary.
