Job Summary:

/n An MDSCoordinator Registered Nurse (MDS RN Coordinator) is responsiblefor coordinating and managing the comprehensive assessment processfor residents in long-term care facilities. The MDS RN Coordinatorensures compliance with regulatory requirements, accuratedocumentation, and the development of care plans that address themedical, functional, and psychological needs of patients in skillednursing facilities (SNFs). /n/n

Key Responsibilities:

/n /n

• Coordinate the completion of the Minimum Data Set (MDS) assessmentsfor residents in compliance with federal, state, and localregulations. /n
• Ensure the accuracy and timeliness of MDSdocumentation, care planning, and assessments for residents inskilled nursing facilities. /n
• Collaborate with interdisciplinaryteams, including physicians, therapists, social workers, andnursing staff, to develop and implement individualized care plansbased on MDS findings. /n
• Review and analyze clinical data,including diagnoses, medications, functional status, and nursingcare needs, to identify areas for improvement in patient care. /n
• Monitor resident care to ensure quality outcomes and adherence toregulatory guidelines, including reimbursement requirements. /n
• Identify and communicate issues related to Medicare/Medicaidreimbursement, documenting and ensuring compliance with paymentcriteria. /n
• Educate staff on MDS guidelines, coding, anddocumentation requirements, promoting adherence to regulatorystandards. /n
• Develop and maintain documentation and reportingsystems that meet regulatory requirements and support datacollection for performance improvement initiatives. /n
• Ensurethat care plans are accurately documented, updated, and reflect theresidents’ current needs, functioning, and goals. /n
• Serve as aresource to the interdisciplinary team for clinical questionsrelated to the MDS process and patient care.

Qualifications:

• Education:

  Bachelor of Science inNursing (BSN) preferred; Associate Degree in Nursing (ADN)accepted. /n

• Licensure:

  /n /n

• Valid Registered Nurse (RN)license in the state of practice. /n /n /n

• Certification:

  /n /n

• MDS Coordinator certification (ACM, RAC-CT, or CRRN) preferred.
