Ft. Pierce, FL We are seeking an experienced and compassionate Social Worker (MSW or LCSW) with a strong background in hospital acute care case management and discharge planning. Required Experience:

2 years Acute care hospital case management 2 years Hospital discharge planning and execution Familiarity with MDR/IDT processes Proficiency with

MIDAS/Meditech, Navihealth, and PASRR Active MSW or LCSW Preferred Qualifications:

Prior travel contract experience Ability to work independently in a high-volume setting Flexible and adaptable to changing priorities About the

Hospital:

A full-service acute care hospital with 344 beds (excluding Behavioral Health and Acute Rehab). You’ll play a critical role in helping patients transition safely and efficiently from hospital to home or the next level of care.

What You’ll Be Doing:

Conducting discharge planning and managing complex patient cases Participating in multidisciplinary rounds (MDR) and care coordination Communicating with patients and families to support informed transitions Facilitating referrals and linking patients to community resources Documenting in hospital systems including

MIDAS/Meditech, Nate, Navihealth, and Care Team Assignment Unit Details:

Patient Ratios:

Complex Case Management (with Matrix RN): 1:8-9

Floating Policy:

All Social Workers float except those assigned to

Trauma Documentation Systems:
