The Columbia Presbyterian Community is seeking a highly skilled and compassionate Director of Social Services to join our team to create a warm hospitality experience for all our residents, staff and visitors. In this position, you will enjoy working with a passionate team to provide quality service throughout our community.
What You’ll Do:
Facilitate trust, initiating assessment of resident and family psychosocial status, coordinating and clarifying patient-specific financial aspects of care, and collaborating positive linkages with referral sources. Admit residents following the appropriate policies and procedures outlined by state, federal and organizational regulations and guidelines. Establish and maintain waiting list, ensuring that applicable state, federal and organizational regulations and guidelines are followed. Assist residents with their adjustment to their new living conditions and community. Serve as a first point of contact related to concerns of residents and/or family members; initiate resolutions. Conduct SNF/Rehab prospect tours and visitor tours and provide comprehensive information about the community. Encourage family contact and communicate with families on an as needed basis; assist residents with various forms of communication to family.
What We Are Looking For:
Associate’s degree in Social Services or related field. 1 year of long-term care experience. 1 year of Medicare admission experience.
