Title:
Peer Support Specialist (Transition to Community Living)
Program Overview:
TCL Community Inclusion Peer Support Specialists supports participants in their goal of becoming part of the community for sustainable recovery. Members are linked to local behavioral healthcare providers in their community to support whole-person care. The program assists members in learning everyday skills, taking part in community activities and sharing their lives with family, friends, and neighbors. TCL also helps people identify their interests and connects them to supported employment.
Position Summary:
A full-time position supporting Trillium Health Resource members enrolled in the TCL program. These members are currently in transition in their lives and are working to strengthen their network of support in the community. Primary duties will be supporting individuals in their transition to independence and sustainability. PSS will provide support to members as they progress in recovery, independence, self-sufficiency goals, or stabilizing health (behavioral or medical), through behavioral health services, community medical providers and local resources through a Continuum of Care with the Trillium Health Resources local management entity/managed care organization.
Responsibilities :
- The Peer Support Specialist will be responsible for intentional outreach for difficult to engage members in order to provide support and linkage to community resources.
- Provide peer support services that will help members explore interests and goals and will provide transportation and connection to community inclusion activities that match their aspirations.
- Facilitates the exploration of vocational or educational goals with members and connects them to IPS-SE, to include benefits counseling.
- Conducts biopsychosocial assessments in person with members to identify needs in key areas (e.g., housing, healthcare, employment, financial assistance, benefits, social connection, etc.).
- Coordinates with members’ assigned care managers on a regular basis to support collaboration between ABCCM and Trillium Health Resources in a holistic approach to services.
- Makes referrals to network providers to address identified needs based on understanding of eligibility criteria, capacity, location, and client preference.
- Maintains a working knowledge of Network providers’ menu of services, eligibility criteria, intake process and preferences, specifically in Housing, Employment and MH/SU systems.
- Facilitates community integration and continuity of care by ensuring that the member has enduring ties to their community and support systems during critical transitional periods.
- Provides Case Management with members to assure proper services and needs are being addressed at appropriate times, per TCL program guidelines.
- Documents all activity in accordance with program guidelines and in compliance with internal policies and procedures. (Utilizing the Trillium Health data management system.)
- Participates in all appropriate staff, supervision, and training meetings and assumes other duties as assigned by Senior Management.

- Strong organizational skills and attention to detail.
- Proven ability to perform at a high level in a fast-paced, changing environment
- Experience working with individuals with mental health/substance use disorders.
- Must have knowledge of clinical issues, skill in community resource development, be well organized, a self-starter, and able to work as a member of a team.

Qualifications:
- Must be a NC Certified Peer Support Specialist, or obtain certification within 60 days of hire.
- High school diploma or equivalency is required for this position but preference will be given to BS in Human Services
- Lived experience with MH, SU or I/DD issues with receipt of services and supports as a consumer or family member is desired.
Existing knowledge of the area resources available preferred.
