In addition to the mentioned, VNA Alliance offers additional medical services and implemented a Safe at Home program that is a clinical pathway to optimize outcomes in the acute care and home health care setting. The Intake Assistant is responsible for supporting the clinical management team in the daily operations of the Home Health office. Contributes ideas and suggestions to improve techniques, methodologies, and productivity.

QUALIFICATIONS A.

Two (2) years of progressive experience in a home health setting preferred B.

Computer Skills:

Microsoft Word and Excel C. Excellent organizational skills with attention to detail D. Good communication and interpersonal skills E. Ability to prioritize and organize daily work load F. Prior experience in scheduling and/or data entry preferred G. High School education required

DUTIES AND RESPONSIBILITIES A.

Prepares and runs reports from the computer system and ensures information is forwarded to the proper individual. B. Creates/updates/maintains spreadsheets. C. Phone calls to patients to enhance patient experience. D. Assists with Face-to-Face process. E. Responsible for clearing faxes and distributing to correct individuals. F. Assist with clerical duties associated with referrals G. Enter patient demographic information into the database H. Assist in statistical data gathering I. Prepare weekly referral report J. Other duties as assigned
