Position Overview:

The Intake Coordinator will be the first point of contact for incoming patient referrals and play a vital role in coordinating care for new clients. You will manage multiple referrals in various stages, ensure accurate documentation, and collaborate with our staffing team to match clients with qualified and compassionate caregivers.

Key Responsibilities:

Manage daily incoming patient referrals from hospitals, case managers, insurance providers, and family members. Maintain a referral pipeline and accurately track each case through intake stages using internal systems. Communicate effectively with patients, families, and referral sources to gather all necessary documentation and care requirements. Assess patient needs and preferences to match them with appropriate caregivers based on skills, availability, and compatibility. Collaborate closely with the staffing and HR departments to ensure timely service initiation. Follow up on pending referrals and ensure all intake documents are completed and compliant. Maintain up-to-date knowledge of service offerings, payer requirements, and regulatory guidelines. Support the transition of new patients from intake to active service smoothly and professionally. Staff all cases assigned and ensure the move within the pipeline in a timely manner.

Qualifications:

Previous experience in healthcare intake, scheduling, or coordination (home care experience strongly preferred). Strong organizational and multitasking abilities – must be comfortable managing multiple patient cases simultaneously. Excellent communication and interpersonal skills.
