Job Description
The Ashburnham Westminster Regional School District is seeking a School Adjustment Counselor to promote and support the social-emotional well being of students at Westminster Elementary School.
Duties and Responsibilities
Provide daily individual and/or small group support to students with emotional and mental health needs.
Provide case management, mental health referral, counseling, and crisis intervention for students.
Support students re-entering school following an extended mental health or health-related absence.
Conduct social skill development groups, individual, and/or group counseling.
Provide services to students in accordance with Individualized Education Programs (IEPs) and/or Section 504 Accommodation Plans.
Participate in special education and Section 504 team meetings, IEP and 504 writing, data collection and collection of progress notes.
Teach coping skills, conflict and anger management skills, positive social skills, peer mediation skills, mediation, and resolution skills.
Support the development of intervention strategies and activities that promote Social Emotional Learning and a positive school climate in order to increase success for all students.
Screen and assess students for safety, including conducting risk assessments as needed.
Complete Home Assessments as needed.
Coordinate CRAs (Child Requiring Assistance requests) and 51As with the school principal and/or school leadership team as needed.
Serve as an active member of the school and district crisis team.
Responsible for serving as a referral agent and case manager for students and families in developing a network of social services with community agencies.
Responsible for identification and navigation of outside resources for students.
Collaborate with colleagues, hospitals, other schools, and wrap-around services/community agencies in developing and providing support for students.
Provide consultation and support to grade level teams, administrators, families and local agencies.
Establish and maintain positive working relationships with administrators, clinical team members, the School Resource Officer, school nurse, educators, students, families, and community representatives.
Complete paperwork and reports in accordance with district policies and state/federal laws.
Manage and maintain confidential student records.
Perform other tasks that may be assigned by the Principal or their designee
