The Job Coach will serve as an integral member of the Day Services Team and will assist with the day-to-day activities of the program. They will assist in the implementation of person centered supports in the habilitation process including personal care, daily activities and community-based activities focusing on the participants choices. As a Job Coach you will ensures that quality of care, services and training are provided to individuals in accordance with Federal, State, and local standards. Some of the job duties include: Assist, support, and monitor individuals to enable them to acquire and develop independent living skills and social skills using person-centered planning and self-determination. Ensure the work area is maintained to create a safe and clean environment. Assist, coordinate and transport individuals on activities related to community involvement, vocational programs and other programs or appointments outside the facility. Complete and maintain all documentation regarding participation, progress, programming, health and safety, comprehensive assessments, including daily observation, log sheets, and daily reports as assigned. Completes assessments and questionnaires with individuals for Individual Support Plan meetings and participate in the interdisciplinary team process. Monitor individuals and the surroundings for appropriate safety and health care practices. As well as reports any issues to the appropriate assigned responsible parties. Monitors individuals and their environment for any change in health, mental well-being, and behavioral well-being. Immediately reports any such occurrences or unusual incidents to the appropriate supervisors. As well as, documenting the information in the appropriate places as specified in policy and procedure manuals. Skills and Abilities Ability to work cooperatively with peers and supervisors. Ability to adapt to change. Ability to relate a positive image to individuals, families, and the community. Must have compassion, patience, and ability to nurture. Must be able to read and write American English proficiently. Must maintain training competencies as assigned. Possession of a valid Driver’s License with required endorsement, along with an acceptable driving record for the past three years is required to transport. Must be willing to undergo a background and drug screening Education and Experience High School Diploma or GED required. Verifiable previous experience working with intellectually and developmentally disabled adults is preferred. Physical Requirements Ability to work in close physical contact with people for providing personal hygiene assistance to individuals who cannot provide for themselves. Must be physically capable of transferring and lifting persons who require physical assistance with or without reasonable accommodation.
