As a Supported Living Coach, you will provide one-on-one coaching and practical support in daily living skills, housing stability, financial literacy, and social development, helping individuals successfully reside in their own homes and communities. This is a rewarding role that directly impacts lives and fosters long-term independence.
Duties:
Assist individuals in locating and maintaining safe, affordable housing Support the development or enhancement of daily living skills, such as personal hygiene, grooming, and household chores Provide hands-on assistance with meal planning and preparation, budgeting, shopping, and managing personal finances Teach and reinforce adaptive and social skills necessary for independent living Promote health, safety, and overall well-being in a community-based living environment Collaborate with families, case managers, and interdisciplinary teams to develop and implement individualized support plans Maintain accurate, timely documentation in compliance with agency and Medicaid/APD standards Qualifications (Must meet one of the following): Bachelor’s degree from an accredited college or university in Education, Rehabilitative Science, Business, or a related field Associate’s degree or 2 years of college and 2 years of direct experience supporting individuals with developmental disabilities One year of college and 3 years of direct experience with the target population A minimum of 4 years of documented, direct professional experience working with individuals with developmental disabilities (can substitute for college on a year-for-year basis)

Strong communication and interpersonal skills Ability to work independently and problem-solve in real-time Reliable, trustworthy, and organized Flexible availability for PRN scheduling needs Familiarity with Medicaid Waiver services and APD documentation practices Advance computer skills
